CLINICAL ASSESSMENT PROGRAM (KID'S FIRST)

Program Cost: $380.00
Client should submit payment for initial drug screen prior to reporting to Acadiana Safety. Client shall appear at Acadiana Safety with valid state or federal picture identification (ID) card, $120.00 screen fee, a $5.00 dollar collection fee per sample, and submit to one or more of the following:
  • 10 panel hair/10 panel urine ($120.00);
  • steriods, synthetic marijuana, bath salts, hair EtG alcohol, or PEth for alcohol
    (see Acadiana Safety Fee Schedule)

    1st Appointment Requirements

  • A pictured ID is required.
  • Full Payment in the form of a check or money order or PayPal is required.
  • The following items are required at your initial appointment:
    _____ Completed Bio-psychosocial Questionnaire
    _____ Completed Collateral Consents (2)
    _____ Pharmacy records, for any prescribed medication within the last twelve (12) months
    _____ Any medical records concerning alcohol or drug treatment within the last five (5) years

    *Failure to meet the requirements shall result in a missed appointment.

    What to Expect While in the Evaluation Process

    • Participant meets with three (3) Licensed Clinicians for a total of three (3) appointments.
    • Participant attends face to face interviews where written and verbal assessments are completed.
    • Upon receiving all data collected, recommendations are made to the participant during an exit interview.
    • The Court is continually kept informed regarding Compliance/Non-compliance issues and successful completion.

    Rules Regarding Client Conduct

    • No Drugs or Alcohol allowed on premises.
    • No food or beverages while in counseling session.
    • Turn off all mobile devices.
    • DO NOT bring children.
    • DO NOT bring in documentation regarding other parties involved.
    • DO NOT come to the assessment under the influence of drugs or alcohol.
    • Participant will be assessed a $75.00 fee for missed or cancelled appointment without a 24 hour notice. A Non-Compliance report to the court will be generated. This will also delay assessment results from being completed in a timely manner.
    • Medication bottles ARE NOT ACCEPTED as a substitute for pharmacy records.


    Re-screen: Initial drug screens older than three (3) months, shall require repeat screening at client's cost.

    Re-assessment: Evaluations between three (3) and six (6) months old shall require a re-assessment at a cost of $180.00 + $120.00 screen fee at Acadiana Safety.. Evaluations older than six (6) months shall require a full assessment at a cost of $380.00 + $120.00 screen fee at Acadiana Safety..

    Expert Testimony: If any party desires to subpoena CLEARSTART, LLC to testify at any hearing, the subpoena shall be directed to April Micheau, LCSW at CLEARSTART, LLC's address. CLEARSTART, LLC will charge an expert witness fee of $350.00. Expert testimony fees are to be paid within forty-eight (48) hours of the scheduled hearing date.
